If your child/children attended Sullivan School District last year, you will be required to register your child/children for the 2024 - 2025 school year through Skyward Family Access.

In person registration will take place in each building on July 24th and July 25th during the hours of 8:00 a.m. to 3:00 p.m.

Registration will open online July 18, 2024 and will be available online through August 14, 2024. As you register your child please review all information in the student and family section to verify that it is accurate. Once you have completed all of the steps to online registration you will have the option of paying your fees through RevTrak or you can bring in a payment to your child's school.

Simply log-on to your Family Access account and click on the "Online Registration" button. Once logged in, you will need to complete the listed steps no later then August 14th, 2024 for all students in your family.

If your child is new to Sullivan Schools you will have to come in during registration to enroll your child for the 2024 - 2025 school year.

Required Physical Exams: Parents are reminded that students entering Kindergarten, sixth and ninth grades are required to have a physical examination prior to the start of school. Students in Kindergarten and students new from out of state must have a vision exam by October 15.

Dental exams are required for grades Kindergarten, Second and Sixth and must be submitted by May 15.

Students participating in sports must have an athletic physical completed by a licensed physician and must be turned in prior to tryouts or the the first day of practice if there are no tryouts. This physical is good for one year from the date of the exam.

Physical examination forms are available at all the schools and may be picked up at any time. Health forms can also be found in the Health section of the District website.

Sarah Bush Lincoln Dental Services (Dental Van): Sarah Bush Lincoln Dental Services is pleased that they will be coming to your child's school throughout the year. All children are eligible to receive oral education and a dental exam at no cost. The program provides full dental services at no cost to the children that meet the eligibility requirement. 9-12th Grade Student: Lunch Price is $3.00 and Breakfast Price is $2.00

There are no athletic fees this year
